With a basis in Scotland’s National Dementia Strategy, North Lanarkshire’s ‘Dementia Demonstrator Site’ is delivering better outcomes for and with people living with dementia and their carers through a range of support, care and treatment.
Led by a genuinely multi agency team, the project works closely with a wide range of partners through the North Lanarkshire Health and Care Partnership and the Older People’s Partnership Board. Amongst its innovative work, the project has recognised that people with dementia can often become socially isolated, and has worked to support people within their community to boost not only their health, but their social inclusion. That proactive approach is making a real difference to the lives of people affected by dementia and led to Motherwell becoming Scotland’s first dementia friendly town.
The unique approach is leading the way in integrated and inclusive help for those living with dementia, and is already helping others transfer that learning into communities across Scotland.
